? Teacher’s Healthcare Management Jule S. Mini looked out of the window and forecasted that there will be rain soon. Sitting in her own office space, she was deliberating on the managerial processes that were required for the business of her new client. Mini worked at Instill Co. that provided consultation services to its clients and currently came back to her office after meeting Dr. Froju Yen. Dr Yen was a physician who had split from a large group to form a solo practice and he had already secured an office space prior to contacting Instill Co. For Mini, her job was to identify the managerial processes that must be implemented, designed, and completed prior to opening day. Till now, she had identified several processes, like financial management, human resources, supply chain management, governmental compliance, and credentialing that would work behind providing Dr. Yen his dream solo practice. However, the main challenge in this situation was to assure that the practice is ready to operate in 60 days. With no ray of sunshine coming from the sky, Mini hoped to find a lightning of creativity to identify managerial processes that must be implemented, designed, and completed prior to opening day. Mini set to work immediately and started with the background. FroYen Clinic is a clinic that is developed and made for those individuals who need services for a doctor in the area of XYM. The service that is to be set in motion in the coming 60 days has set its focus on those individuals as well as patients that need an affordable, reliable, and clean place for the cure of their illness or problems. The market identified therefore begins from low end and continues till the high end market where more preference is on those patients who have enough money for healthcare. In order to set the operations of FroYen Clinic in motion, the main objective is to begin a service for those individuals who have income for healthcare. With that established, the service also aims at keeping the operations in motion so that the clinic functions without loss. After identifying this aim and objective, it is important to pinpoint the mission for the clinic. The mission developed for Dr. Yen’s practice is to establish a practice that is well known and well respected by the patients and the health care community alike through quality of service. With the help of Dr. Yen’s best physician practices, thereby depicting the service, maintenance of the quality standards, establishing a comforting environment for the patients, and the activation of strong managerial practices, the confidence of the targeted market would be easy to gain. Zwith the amount of revenue generated over the years, the clinic will also expand by purchasing the relevant machines. The financing will be carried out from the initial capital and loans. For FroYen Clinic to be operating in the health sector environment would mean adding a huge contribution to the already existing infrastructure. This step will therefore reduce the influx of patients for other hospitals at the XYM area. The established clinic will hence, possess sufficient resources and professional conduct of the physician that will work as an advantage for the FroYen Clinic as well as the patients most of the times. The clinic will be following a result based performance evaluation framework so that the inefficiencies can be soundly addressed to ensure greater quality. A solo practice faces a number of challenges but developing strategies to ensure availability of quality health service will work towards decreasing those challenges. FroYen Clinic will therefore work as a means to curing illness through the use of efficient medical treatments and will commence its operations in the coming 60 days. The clinic will start on July 10th 2012 and will be established at the XYM Place Nth Ave with one physician that is Dr. Yen, a physician assistant, a nurse practitioner, and a receptionist. The one Line Pitch identified for FroYen Clinic is therefore: “Because We Care for You” Supply Chain Operations For Dr. Yen’s physician practice to be efficient and smooth, it is necessary to identify the related supply chain operations where the input, processes, and output play a very important role in making the clinic successful. Input: Suppliers: To begin with, the suppliers for the machines will be outsourced. Since the office space has already been purchased. The suppliers in this case will be able to provide the clinic machines, office supplies, office stationary, and advertisement assistance. Beginning with the machines, where some of the important tools for the clinic will be purchased, it will also be made sure that some of them are leased. The suppliers will be provided quotas and negotiation will be conducted over the price so that cost efficiency is also maintained. In order to install the required tools, help from labor will be taken if machine setup is not provided from the suppliers. As for the office interiors, it will be directly purchased from the suppliers and will be paid after the purchase is made. Other than that, the process and dealing with the suppliers of office stationary will also follow the same approach. For example the things needed for the office includes the communication technology such as Wifi, laptops and mobile, and fax facility for the office. It will ensured that the ADAAG clause is met in order to ensure the accessibility guidelines for building and facility are met (CDIHP, 2009) The employees: In order to provide the best treatment to the patients, the physician and the other staff like the physician assistant, the nurse practitioner, sweeper, guard, driver and the receptionist/secretary will be fully trained; they will possess an up to date knowledge about their jobs and its related activities. In case a need for training arises, it will be ensured that training from reputable agencies is attained, which will also be outsourced. In order to achieve the best standards in every activity, regular assessment of performance and skill will be conducted. The employees form an integral part of the supply chain because the service travels from person to person and ends after the patient is healthy. Overall operations in input: For FroYen Clinic, the operations developed are straightforward, valuable and cost efficient. The starting point for any operation is the purchase of the land as it forms the basic factor of production. In this case, it has already been bought so the next step is to ensure that inputs such as the ones previously defined and the ones like the management execution, logistics, financial management, and monitoring of the information system is achieved. The goal is to ensure that the services that directly reach the patients actually exist. Health and safety clauses will also be ensured, which means that the clinic would be operating in a germ free environment. So, the entire place would be again sterilized and cleaned (ACT, 2011). Operations: The next step in the supply of chain of this clinic is the operations phase where several strategies to set the health services in motion will be adopted. In this phase, services such as promotional services, preventive services, and curative services will be carried out that will be a way of delivering the product and services of the clinic. The clinic will mostly focus on those patients that have a general population of illness; those with special need will be referred to the specialist. Source: Department of Health (2008) The reception: The service begins the moment a client enters the clinic. For example the indoor environment created for the clinic will be fresh and relaxing at the same time, mute and shades of blue and green will be used that will enhance the calming vibrations for the entire aura. The receptionist/secretary would maintain an eye contact with the entering patient and will welcome them with a greeting. A number would be given to the patient. However, if it is an emergency this step would be skipped and the physician would provide the required treatment. The clinic will follow the rule of customer centricity and will leave no stone unturned in providing quality services to those in need of it. Information database: The record of every client will be saved on the information database present at the clinic. It will be the responsibility of the receptionist/secretary to maintain this database where the day, date, time, reason for coming etc. will be recorded. In this way, the history of the patient will show up to the doctor as he or she enters the clinic. Prescription and other: The last stage of the process is to examine the patient, provide them the remedial action to be taken, and then bid them farewell. It will be made sure that the service provided is exceptional as this process is the sole reason of this clinic’s existence. Output: Follow up: Most of the time the patient himself provides a feedback but if this doesn’t happen, Dr. Yen will ensure a follow up which will be carried out by his secretary if not personally. The FroYen clinic believes in establishing long lasting relationship with its clients and will therefore use the information database to aid the follow up meeting or call. The output methods will therefore asses the intermediate effects on the patient; evidently their compliance with the prescribed medicine is a factor entirely dependent on them (ReadPeriodicals, 2010) Evaluation: As for the rest of the staff, other evaluation techniques shall also be followed that would assess the performance in terms of cost effectiveness, quality, sustainability, and fairness. With the supply chain and its requirements identified, Mini was interrupted because of some calls and lunch break. She made a quick conversation with Mike from finance and Angela from the HR department, since she needed the information for the process requirement of these processes. After returning from the break she set again to work on the managerial process required in other areas. Human Resource: Mini has identified that the management team of the FroYen Clinic consisted of Dr. Yen as the CEO and the head physician with physician assistant, the nurse practitioner, sweeper, guard, driver, and the receptionist/secretary as the rest of the employees. The attendance, salaries, and training needs shall be carried out by Dr. Yen himself where the following training shall be kept in mind. If an employee is late three times a month then the fourth late will deduct a specific percentage from their salary. The lunch break will be for one hour and the employee must return in time as this is a desk job except for the driver. The management team will increase with time depending upon the need of the operations as months pass by. The communication pattern will therefore be decentralized. The organization would begin with the capital of the CEO as funds which are around $10,000 by the owner and a loan from the bank which will be returned in the coming two years. The Financials The clinic will start up with financing being carried out through the personal investment and a business loan taken from the bank that will be short term or long term in nature, depending on the terms and conditions set by the bank. With greater risk, there is always great return so Dr. Yen will provide personal guarantee for the loan. Basically, the clinic management will make sure that those who pay the clinic with money in order to achieve the services, are provided with exceptional service. The issues such as the reimbursement, grant accounting, and OSHA along with licensure and due diligence will be ensured so that those factors that violate the law are controlled and identified (HCCA, 2012). The managerial process will have the following objectives to make sure that the government compliance is met: 1. To ensure practices that encourage quality compliance programs where introduction, development, and maintenance will act as the best approach for compliance. 2. Continuously monitor and interact with the government agencies so the information regarding high quality compliance programs is exchanged. 3. To makes sure that training is provided after identifying high quality educational opportunities so that compliance is ensured in term of government requirements. It had been three days since the final meeting held with Dr. Yen and the following day was due for the presentation. When Mini closed her file after proof reading all the documents and left her office today, the sun had set but the rain she had anticipated didn’t appear. The sky was clear and today’s agenda went smooth. As she drove back home and thought about the sequence of the presentation, she hoped that it doesn’t rain tomorrow too. Dr. Yen had his hopes high for this report and so did her boos. They knew she could do it, she wished her presentation to go as smooth as today.
